Following recent changes to the Guild’s Privacy Policy members are asked to complete the following Membership Declaration and Data Privacy Consent forms.
Either fill in the online forms below or scroll down and download the paper version, complete both sections and upload it using the “upload” section.
Junior (under 18) members should complete the Junior Membership Declaration, here, which also has a parental consent section
If you are having difficulty in completing this online form, please contact your Branch Secretary for advice.
If you have completed the online form above you do not need to use the paper forms.
